Grounds Condition Assessment
This assessment tool is to be used by the Carnival Grounds Committee both prior and during the carnival as required. The Carnival Grounds Committe shall consist of a representative from the host club, the senior umpire at the carnival and the senior horse welfare officer at the carnival. Any deterioration or change in the conditions will require the completion of the form again during the event.
Submission of this form will send notification directly to the SEO for record keeping.
